The following are recommended "Best Practices" that will help you achieve the best experience during your doctor office visit:
1. When you contact a provider to set your appointment it is critical that you inform them that you have a "PPO plan". If the provider requests the name of the specific PPO plan you should respond per the following:
"I have a National PPO plan consisting of several preferred regional networks."
OR
"My PPO plan is (network name)."
Always reference the PPO Network's name as the provider's office may not recognize your program's name. An example of where you will find the PPO Network that the provider participates in is highlighted below.
NAME: Advanced Physical Therapy
ADDRESS: 7175 West Oakland Park Blvd., Ft. Lauderhill, FL 33313
PHONE: (954) 572-0777 - Map and Directions
SPECIALTY: PHYSICAL THERAPY - PPO NETWORK: First Access PPO
IMPORTANT: First Access consists of "preferred" contracted providers. For best results, always reference "First Access" when making your appointment as the provider's office may not recognize your program's name.
2. Always be prepared to present your ID card as well as provide payment for your portion of services received at the provider's office.
3. Always bring the "Dear Provider Letter©" with you to your appointment.
